Propuesta terapéutica para un paciente adulto mayor pluripatológico

Abstract

The present investigation was oriented to the establishment of a therapeutic proposal for a multi-pathological elderly patient, being that person, generally of advanced age, who presents three or more chronic diseases causing a reduction in autonomy and functional capacities. In the present case, it is a 70-year-old man who suffers from high blood pressure, type II diabetes mellitus, heart failure, asthma and polyarthrosis. Therefore, the general objective is: To propose a treatment, through a review of treatment guides and therapeutic follow-up, for an adequate management of the multipathological patient and improvement of his current condition. The methodology used consisted of a bibliographic research on treatment guidelines, therapeutic follow-up and updated scientific articles. The identification of the different conditions of the patient allowed establishing a medication plan based on a combination of ACE inhibitors (Ramipril) + thiazide-type diuretics (Hydrochlorothiazide) to treat arterial hypertension and heart failure, while drugs were recommended for type II Diabetes Oral hypoglycemic drugs such as Metformin and Linagliptin, which belongs to the dipeptidyl peptidase 4 inhibitor drugs. Regarding asthma, a combination of corticosteroid inhalers and salmeterol was prescribed. On the other hand, for polyarthrosis, topical NSAIDs such as diclofenac and analgesics such as paracetamol were established in order to reduce joint pain. For proper adherence to treatment, it was proposed to change the patient's attitude by informing him and his family correctly about his treatment; and, fundamentally, with a healthy eating plan.
