Descripción : Bovine mastitis is one of the main health problems that small and medium producers have in their dairy herds, this is due to the conditions and the medium that performs this work and the lack of knowledge in the management of this activity. Considered one of the most important diseases that affect bovine milk cattle is bovine mastitis which is the inflammation of the mammary gland whose origin can be infectious, traumatic or toxic; being a pathology recognized worldwide for causing great economic losses to both the producer and the industry. It is estimated that between 15% and 20% of cows in a dairy herd are affected in some way with mastitis in one or more mammary rooms. The great majority of cases of mastitis occur in a subclinical manner, so that the affected animals do not present any type of symptomatology, however these alterations are reflected in low levels of production and the milk presents poor sanitary and organoleptic conditions. The objective of the present investigation was to determine the antimicrobial effect of lidocaine hydrochloride via the intramammary route in subclinical bovine mastitis in animals of the Granja Santa Inés - Machala. Of a total of 133 milk samples that were analyzed by the CMT test, 30 were positive for mastitis, selecting 21 for being the most highly reactive (++ and +++) that were subjected to a microbiological study to identify the pathogens that intervened and know their sensitivity and resistance to antibiotics. The affected mammary rooms were subjected to an alternative treatment, applying 4 cm of 2% lidocaine without epinephrine + 16 cm of ringer's lactate, in a total of 20 cm of intramammary solution for each quarter affected, resulting in a lidocaine hydrochloride solution at 0%. , 4% for a period of 7 consecutive days post milking once a day. At the end of the treatment, the CMT and the bacteriological culture were repeated to know the effect of the treatment. The results determined that the incidence of subclinical bovine mastitis in Granja Santa Inés was 22.56%. The degree of bovine mastitis to CMT evolved from 52.4% with +++ and 42.9% from ++ before treatment to 28.6% with + and 23.8% ++ after treatment. The causative agents found with the highest incidence before treatment were Staphylococcus (non-aureus) (52.38%) and Streptococcus uberis (28.57%) and after treatment Streptococcus dysgalactiae (23.81%) and Streptococcus agalactiae (9.52). %). Sensitivity to antibiotics such as amoxicillin + clavulanic acid (100%) and cefquinome (95%) as well as resistance to cloxacillin (60%) and lincomycin (50%) before the treatment was confirmed after its application, reaffirming the sensitivity of 100% for amoxicillin + clavulanic acid, cefquinome, sulfatrimetropim, cephalexin and penicillin and resistance (12.5%) for lincomycin, cloxacillin and ampicillin. The antimicrobial effect of lidocaine hydrochloride as a treatment for subclinical mastitis (61.09%) was not statistically significant, so it is rejected. It is recommended, given the results obtained, that lidocaine hydrochloride be considered an alternative treatment of a curative nature, depending on the causative agent, while not contributing to microbial resistance and being of low cost, as well as continuing research aimed at the search for new therapies. Applicable to diseases of high economic impact and public health in the area of Veterinary Medicine.
Resumen : La mastitis bovina es uno de los principales problemas sanitarios que tienen los pequeños y medianos productores en sus hatos lecheros, esto es debido a las condiciones y el medio el que se desempeña esta labor y por el desconocimiento en el manejo para esta actividad. Considerada una de las enfermedades más importantes que afectan al ganado bovino de leche está la mastitis bovina que es la inflamación de la glándula mamaria cuyo origen puede ser infeccioso, traumático o tóxico; siendo una patología reconocida mundialmente por causar grandes pérdidas económicas tanto al productor como a la industria. Se estima que entre el 15% y el 20% de las vacas de un hato lechero están afectadas de alguna forma con mastitis en uno o más cuartos mamarios. La gran mayoría de casos de mastitis se da de forma subclínica por lo que los animales afectados no presentan ningún tipo de sintomatología, sin embargo estas alteraciones se ven reflejados en bajos niveles de producción y la leche presenta malas condiciones sanitarias y organolépticas. La presente investigación tuvo como objetivo determinar le efecto antimicrobiano del clorhidrato de lidocaína mediante la vía intramamaria en la mastitis subclínica bovina en animales de la Granja Santa Inés – Machala. De un total de 133 muestras de leche que fueron analizadas mediante la prueba CMT, 30 fueron positivas a mastitis, seleccionándose 21 por ser las de mayor grado de reacción (++ y +++) que fueron sometidas a un estudio microbiológico para identificar los patógenos que intervenían y conocer su sensibilidad y resistencia a los antibióticos. Los cuartos mamarios afectados fueron sometidos a un tratamiento alternativo, aplicando 4 cm de lidocaína al 2% sin epinefrina + 16 cm de lactato de ringer, en un total 20 cm de solución intramamaria para cada cuarto afectado, resultando una solución de lidocaína clorhidrato al 0,4 % por un periodo de 7 días consecutivos post ordeño una vez al día. Al final del tratamiento se repitió la CMT y el cultivo bacteriológico para conocer el efecto del tratamiento. Los resultados determinaron que la incidencia de mastitis bovina subclínica en la Granja Santa Inés fue del 22,56 %. El grado de mastitis bovina al CMT, evoluciono del 52,4% con +++ y 42,9% de ++ antes del tratamiento a 28,6% con + y 23,8% ++ después del tratamiento. Los agentes causales encontrados con mayor incidencia antes del tratamiento fueron Staphylococcus (no aureus) (52,38%) y Streptococcus uberis (28,57%) y después del tratamiento Streptococcus dysgalactiae (23,81%) y Streptococcus agalactiae (9,52%). Se constató sensibilidad a antibióticos como amoxicilina + ácido Clavulanico (100%) y cefquinome (95%) así como resistencia a cloxacilina (60%) y lincomicina (50%) antes del tratamiento para posterior a su aplicación, reafirmarse la sensibilidad del 100% para amoxicilina + ácido Clavulanico, cefquinome, sulfatrimetropim, cefalexina y penicilina y resistencia (12,5%) para lincomicina, cloxacilina y ampicilina. El efecto antimicrobiano del clorhidrato de lidocaína como tratamiento para mastitis subclínica (61,09%) no fue estadísticamente significativo por lo que se rechaza. Se recomienda ante los resultados obtenidos, se considere a la lidocaína clorhidrato un tratamiento alternativo de carácter curativo, según el agente causal, al tiempo que no contribuye a la resistencia microbiana y ser de bajo costo así como continuar investigaciones destinadas a la búsqueda de nuevas terapéuticas aplicables a las enfermedades de alto impacto económico y de salud pública en el área de la Medicina Veterinaria.
