dc.descriptionThe nephrotic syndrome is characterized by alteration of the capillary permeability of glomerular predisposing to the formation of edema, proteinuria results >50 mg/kg/dl or 40 mg/m2/hour, low serum albumin, albumin < 2.5 g/dl, hypercholesterolemia. (3). Estimated annual nephrotic syndrome incidence is 1 - 3 per 100,000 children under 16 years of age and occurs more frequently between 2 and 10 years. (1), being the male greater predominance. (2) , although there are no exact figures for the incidence of this disease in pediatric age group, in Ecuador the Pediatric Hospital Roberto Gilbert Elizalde of the Board of charities of Guayaquil, indicates that kidney disease are very common, and are a average of 15 patients daily with 5 to 6 consultations of patients in hospital in Nephrology. Presents a study describing the clinical features and the process of care nursing executed, in a case of nephrotic syndrome with infant. The results will allow to develop a valid plan of nursing care for the management of patients with pathology. OVERALL OBJECTIVE Describe the clinical features of the nephrotic syndrome in the nursing major served in the General Hospital of Teofilo Davila of Machala in 2017. METHODOLOGY Developed a qualitative research of descriptive, to apply the heuristic, clinical, analytical, synthetic method based on the analysis unit, s and use techniques such as: literature review, document review of the clinical history of the patient, instruments such as: structured categories tab, guides of non-structured interviews to a specialist in Nephrology and Atty.. Head of Pediatric of a case of nephrotic syndrome served in the General Teofilo Davila Hospital during the period November to December 2017. RESULTS: Nephrotic syndrome is a rare disease at global, national and local level, the age of presentation of nephrotic syndrome in children ranges 2 to 10 years of age, as a syndrome of minimal change nephrotic, and less than a year as nephrotic syndrome congenital; our unit of analysis by the age of presentation and the absence of pathological prenatal and post-natal antecedents, nor pathological families, allows us to associate it with the type of idiopathic nephrotic syndrome, without ruling out the possibility of being of type. The clinical features of the nephrotic syndrome in greater infant are, bilateral palpebral edema and upper and lower limbs, coughing without expectoration of moderate intensity, diarrheal stools, with results of proteinuria, dilucional, hyponatremia, severe hypoalbuminemia, hypertriglyceridemia and hypercholesterolemia, nursing care performed revelor weaknesses in the assessment and response to the needs of the patient. CONCLUSIONS In relation to the results, the clinical characteristics of the nephrotic syndrome, the key piece to carry out a process of attention to the disease, the same one that is based on the diagnostic approach with the NANDA taxonomy, where the altered domains were identified, was handled the corresponding NIC and NOC, which has its use and applicability in the field of health.es_ES
dc.description.abstractEl síndrome nefrótico está caracterizado por alteración de la permeabilidad capilar glomerular que predispone a la formación de edema, con resultados de proteinuria >50 mg/kg/dl o 40 mg/m2/hora, hipoalbuminemia, albúmina <2,5 g/dl, hipercolesterolemia. (3). Se estima que la incidencia anual del síndrome nefrótico es de 1-3 por 100,000 niños menores de 16 años de edad y se presenta con más frecuencia entre los 2 y 10 años (1), siendo el sexo masculino el de mayor predominio. (2), aunque en Ecuador no existen cifras exactas de la incidencia de esta enfermedad en edad pediátrica, el Hospital pediátrico Roberto Gilbert Elizalde de la Junta de Beneficencia de Guayaquil, indica que enfermedades renales son muy frecuentes, y se atienden un promedio de 15 pacientes diarios y de 5 a 6 interconsultas de pacientes de hospitalización en nefrología. Se presenta un estudio que describe las características clínicas y el Proceso de Atención de Enfermería ejecutado, en un caso de lactante mayor con síndrome nefrótico. Los resultados permitirán elaborar un plan de cuidados de Enfermería válido para el manejo de pacientes con la patología. OBJETIVO GENERAL Describir las características clínicas del síndrome nefrótico en el lactante mayor atendido en el Hospital General Teófilo Dávila de Machala en el año 2017. METODOLOGÍA Se desarrolló una investigación cualitativa de tipo descriptivo, aplicándose el método heurístico, clínico, analítico, sintético en base a la unidad de análisis, se utilizaron técnicas como: revisión bibliográfica, revisión documental de la historia clínica del paciente, instrumentos como: ficha estructurada de las categorías de análisis, guías de entrevistas no estructuradas a un Especialista en Nefrología y Lcda. Jefa del área de Pediatría de un caso de síndrome nefrótico atendido en el Hospital General Teófilo Dávila durante el período Noviembre y Diciembre del 2017. RESULTADOS: El síndrome nefrótico es una patología poco frecuente a nivel mundial, nacional y local, la edad de presentación del síndrome nefrótico en niños oscila entre los 2 a 10 años de edad, como síndrome nefrótico de cambios mínimos, y menores de un año como síndrome nefrótico congénito; nuestra unidad de análisis por la edad de presentación y la ausencia de antecedentes patológicos prenatales y postnatales, ni patológicos familiares, nos permite asociarlo al tipo de síndrome nefrótico idiopático, sin descartar la posibilidad de ser de tipo congénito. Las características clínicas del síndrome nefrótico en el lactante mayor son, edema palpebral bilateral y de miembros superiores e inferiores, tos sin expectoración de moderada intensidad, deposiciones diarreicas, con resultados de proteinuria, hipoalbuminemia severa, hiponatremia dilucional, hipertrigliceridemia e hipercolesterolemia, la atención de Enfermería ejecutada reveló debilidades en la valoración y respuesta a las necesidades del paciente. CONCLUSIONES: En relación a los resultados, concluimos que conocer las características clínicas del síndrome nefrótico, es la pieza clave para realizar un proceso de Atención de Enfermería eficiente, el mismo que se fundamenta en el enfoque de diagnóstico con la taxonomía NANDA, donde se identificaron los dominios alterados, se manejó el NIC y NOC correspondiente, el cual tiene su uso y aplicabilidad en el campo de la salud.es_ES
